Clinics run Sundays, Starting October 16th at Fitchburg High School
Grades 8-12 from 1:00-2:30   ---   Grades 3 – 7 from 2:30 – 4:00

Waiver of Participation
I know that my participation in sport activities is potentially hazardous and can cause bodily injury. I clearly understand that, by signing this form and/or my involvement in Central Mass Basketball Training programs, I assume all risk for any injury resulting there from. The Central Mass Basketball Program, coaches or training facility is free from fault. I also understand that the camp session fee is non-refundable.
